Practicum Report

Hi Practicum Students,

As you know, your practicum report(s) should be completed, initialed, signed, and submitted by the end of the semester. We have gotten some reports that were not complete. Before you turn in your practicum report, please make sure that the following is done:

In part I, question 5 of the practicum report, it says "Nature and length of three prepracticum field experiences (minimum of 75 clock hours)." In this section, you should list your three fieldwork projects:

Date (ex: Fall 2004), School, brief description (ex: collection development), 25 hours

In part IV, make sure that all dates and initials are filled in. To make sure that you get all the initials that you need, I recommend that you fill in the report at the time of each visit.

Don't forget to have your library teacher(s) sign at the bottom of the form.
