New Student Requirements

For Undergraduate Students:

Incoming undergraduate students are required to submit a completed Health Certificate mailed out at the time of acceptance. The Health Certificate includes a record of immunizations required by the Commonwealth of Massachusetts. If you do not receive your Health Certificate or if it is misplaced, you may download it below. Note that there are three parts:  the Entrance Health Certificate itself, the undergraduate immunization record and the meningitis waiver form.  Please return all 3 forms to the Health Center, 94 Pilgrim Road, Boston MA 02115 or by fax, 617-521-3467.  Be sure to keep a copy of your completed Health Certificate for your records.

For Graduate Students:

All graduate students are required to submit the graduate student immunization record form to the Health Center.  If you are living on campus, you must also submit the Meningitis Waiver Form.  These forms should have been sent to you by your graduate program.  They can also be downloaded from the "Immunizations" section of this web site. Please return form (s) to the Health Center, 94 Pilgrim Road, Boston MA 02115 or by fax, 617-521-3467.  Be sure to keep a copy for your records.

For all students:

The Commonwealth also requires each undergraduate and graduate student to have health insurance.
