New Student Requirements
Incoming students are required to submit a completed Health Certificate mailed out at the time of acceptance, and are required to have a physical examination within the year prior to matriculation. The Health Certificate includes a record of immunizations required by the Commonwealth of Massachusetts. If you do not receive your Health Certificate or if it is misplaced, you may download it below. Please return all forms to the Health Center, 94 Pilgrim Road, Boston MA 02115 or by fax, 617-521-3467 and be sure to keep a copy of your completed Health Certificate for your records.

The Commonwealth also requires each undergraduate and graduate student to have health insurance.
